Student Health and Immunization Requirements

All new students to All Saints are required to have a current physical and immunization record.
  • Pre-K students: A current physical and immunization record that includes:
    • 4 DTP/DTaP/DT vaccines
    • 3 Polio (IPV or OPV)
    • 1 MMR vaccine
    • 3 Hepatitis B vaccines
    • 1 Varicella (Chicken pox)
  • Kindergarten students: A current physical and immunization record that includes:
    • 5 DTP/DTaP/DT vaccines
    • 4 Polio vaccines
    • 2 MMR vaccines
    • 3 Hepatitis B vaccines
    • 2 Varicella vaccines
  • 7th Grade students: A current physical and an immunization record that includes:
    • Tdap booster vaccine plus all of the vaccines listed above 
  • Any 5th grade through 8th grade student that participates in any of the school sports teams must have a current sports physical (or school physical) on file each year.
The Diocese of Palm Beach requires that, prior to attendance in school, each student present or have on file with the school a Florida certificate of immunization for the prevention of those communicable diseases for which immunization is required by the Department of Health. A completed Florida Certificate of Immunization (Form DH 680) is required to document the administration of prescribed immunization doses. An exemption from immunization requirements is permissible only with a physician’s certification as to the need for either a temporary or permanent medical exemption. An exemption from immunization requirements is not permissible for religious, philosophical, personal, or other reasons.
