**All forms must be submitted BEFORE tryouts this year. Please see the website below for the forms that are required. Please do not leave anything blank on these forms as well!

**Every year physicals are an issue. If you are unsure if your physical is valid, please see Nurse Black. Your child must have a valid physical before tryouts as well.
