Attached below is the Voluntary / Optional 24 Hour Student Accident Insurance Program description and instructions. Enrollment in this program is not mandatory. Nothing is to be returned to the school or to our office, please. If interested, please complete the form (page 7) and send a check with the completed form to National Union Fire Insurance at the address specified at the bottom of the form (page 7).
